The Challenge
Limited visibility created safety, maintenance, and asset risks
CoxHealth operates 106 vehicles, including 55 ambulances, across five counties in the Missouri Ozarks. Their EMS teams drive more than two million miles annually, serving scattered communities where crews routinely travel long distances between calls. With more than 400 EMS personnel, fleet uptime, vehicle safety, and equipment availability are mission-critical.
Before Samsara, CoxHealth lacked real-time insight into fleet operations. Safety incidents were difficult to investigate or defend, leaving the organization exposed to risk and potential litigation. Maintenance tracking was manual and time-consuming, pulling leaders away from higher-value work and making it difficult to plan vehicle replacements. Across a large service area, maintaining real-time visibility of ambulances and critical EMS equipment could be time-intensive, particularly when verifying asset locations and readiness. Without connected data, CoxHealth struggled to proactively manage safety, costs, and operational efficiency at scale.
"By choosing Samsara, we're able to save money and follow through on our commitment to the public. We've drawn in recruits from nine different states—people want to come to work for CoxHealth, and they'll move to work for our EMS agency. That is so freaking cool."

The Solution
Rolling out Samsara to connect EMS operations end to end
CoxHealth implemented Samsara's Connected Operations platform as their centralized EMS fleet management solution. They deployed vehicle telematics and maintenance tools, AI Dash Cams and Multicams, Environmental Monitors, Panic Buttons, Weather Intelligence, and Asset Tags—all integrated through Samsara's API and connected to CoxHealth's computer-aided dispatch (CAD) system.
The platform quickly proved its value in both day-to-day operations and emergency scenarios. To safeguard medications that must be stored just above freezing, Environmental Monitors help CoxHealth protect $3,000 worth of inventory on every ambulance. During a recent winter storm, CoxHealth used Samsara's real-time visibility and Weather Intelligence to safely coordinate patient transfers, reinforcing the importance of connected data during critical events. "Samsara has done wonders for us," said Kyle Meadows, EMS Business Manager, Prehospital Services. "It's well-developed, and the API works seamlessly. The platform has been a big part of creating the positive culture we have today."
Maintenance
Connected Maintenance saves money and reduces processing time by 94%
CoxHealth manages a 24/7 ambulance fleet where each vehicle costs roughly $250,000. Before Samsara, the team spent hours manually processing maintenance invoices and lacked visibility into true vehicle costs, making it difficult to extend the life of ambulances amid supply chain delays. With Samsara's AI-powered Connected Maintenance, CoxHealth centralized fleet data, eliminated manual work, and gained real-time insight. To date, the organization saved a significant amount of money using total asset and cost visibility to drive smarter maintenance and replacement strategies.

Cameras & Video
360-degree visibility protects EMS crews, patients, and budgets
CoxHealth uses Samsara AI Dash Cams and AI Multicams to create a 360-degree view around and inside their ambulances. Dash cams provide real-time behavior alerts and enable driver exoneration, while Multicams capture side, back, and patient-compartment views. Together, the cameras help CoxHealth quickly resolve claims, protect crews from false allegations, and gain full context during complex emergency responses. The result is a safer, more defensible EMS operation with clearer insight into every critical moment.

Asset Tag
Real-time asset visibility that keeps ambulances in service
CoxHealth uses Samsara Asset Tags to track high‑value medical equipment across their 6,000‑square‑mile service area. By gaining real‑time visibility into critical asset locations, such as ambulance cots and EMT go bags, the organization aims to reduce equipment loss, avoid unnecessary downtime, and keep ambulances in service.
